Registration times for new students is Monday-Friday 8:30 a.m. - 1:00 p.m.
 
 

Required Documents for Registration

1) Birth Certificate

2) Georgia Certificate of Immunization.  Records from other states must be transferred to a Georgia Immunization Certificate, Form 3231, completed by the health dept. or a private doctor.  Required shots include Hepatitis B, DPT, polio, HIB, two MMR shots and two chicken pox vaccines or year of chickenpox illness noted on certificate.     

3)  Vision, Hearing & Dental Certificate on Georgia Form #3300, completed by a private physician or the Health Dept.

4)  Proof of residency: house lease or deed or non-contingent sales contract with one utility bill, OR apartment lease with one utility bill.  If the enrolling parent is living with a friend or relative, a Residency Affidavit (available only at school) signed and notarized, accompanied by an apartment lease or house deed and one current utility bill will be required.  (Utility bills accepted are for either gas, water or electricity.)

5) Photo ID and proof of custody or guardianship, if you are not the birth parent.  Only natural parents and/or legal guardians can register children.

6)  Withdrawal paperwork from previous school including current grades, attendance record, and discipline record. If the student is receiving special education services, a copy of a current IEP must be presented at time of enrollment.

International Newcomers Center

If your child has never attended school in the United States, or has been in an ESOL program in another state for less than 2 years, please call the Gwinnett County Public Schools International Newcomers Center, at 678-985-5200 for an appointment (for a math and reading test) before registering at Bay Creek Middle School.  Then, bring all paperwork from the Newcomers Center to our school.
